ALL RECORDS AND PAPERWORK ALONG WITH A COPY OF YOUR PHOTO ID AND INSURANCE CARD(S) MUST BE SUBMITTED 72 BUSINESS HOURS PRIOR TO YOUR APPOINTMENT

Please note: if we have all the paperwork returned as soon as possible, you are moved to the top of the waitlist and if we have a cancellation, it is likely we could move your appointment sooner.

It is important that we have the most accurate data for your visit.

For the medications list:

• Please look at your medications bottle and copy the name, dosage and dosing instructions.
• Please include any supplements with the dose and how often you take them.

For lab and imaging results: We will request these from your provider, but it is helpful if the patient also calls the provider to ask them to send over the results. This will ensure that we have your information for the visit. Have the results faxed to (847) 706-9891.

For Diabetics:

• Submit your blood glucose logs, continuous glucose monitor (CGM) or pump data via fax at (847) 706-9891 or email  OFFICE@ADVANENDO.COMfor the provider to review.
