Test Results

Unless the Provider tells you differently, all test results (lab tests and imaging) will be discussed with you at your next appointment. If you are anxious about your results you are welcome to call the office and ask if there is a sooner appointment for you to discuss with the Provider.

If you are asked to call for test results, it is the patients responsibility to call for the result.The Provider will then return your call within 48 hours from the time of the call.

Appointments and Scheduling

We are typically booked out 4-6 weeks for routine follow up appointments. Therefore, it is best to schedule your next visit at the end of each visit.

Appointments must be cancelled MORE THAN 24 BUSINESS HOURS in advance. This means that appointments for Monday must be cancelled no later than 5:00pm CST the Friday before. If the appointment is not cancelled before this time or the appointment is missed, there will be a fee charged to the patient.

Maintaining a relationship with your PCP

We ask that you maintain a relationship with your primary care physician.

Our office does not manage internal medicine issues and there are some forms, such as disability forms, that we are unable to complete as a specialty care provider. Your primary care physician may also be able to see you on short notice if you have an acute illness. If you do not have a primary care physician, we would be happy to recommend one.

Medication refills and CMN forms

To get your prescription filled, please call your pharmacy and ask them to fax us a refill request. *Strongly consider having your medications placed on automatic refill at your pharmacy.

For Medicare CMN (Certificate of Medical Necessity) forms for diabetic supplies, these form must come from your pharmacy as these are patient specific and cannot originate from our office.

The Provider will respond to the pharmacy for medication refills and CMN forms within 48 business hours. Please take this into account so that you do not run out of medication/supplies. We never want to see a patient run out of their medications or supplies.

Prior Authorizations

We complete prior authorization when clinically appropriate.

Please note that prior authorizations can take 10+ business days for the insurance to process.

If there is an issue with running out of medications prior to getting the approval, please call our office immediately. We never want to see a patient without their medications

Forms and medical record requests

For patients who need forms completed by our office or requests for medical records, our office requires 5-10 business days to complete these.

There is a $25 fee for printed medical records with an additional $10 fee if the records are then mailed.


Our dedicated staff works to educate and encourage our patients to address chronic medical issues such as diabetes, thyroid issues, polycystic ovarian syndrome, and weight management.
