Please note: if we have all the paperwork returned as soon as possible, you are moved to the top of the waitlist and if we have a cancellation, it is likely we could move your appointment sooner.

For lab and imaging results: We will request these from your provider, but it is helpful if the patient also calls the provider to ask them to send over the results. This will ensure that we have your information for the visit. Have the results faxed to (847) 706-9891.
